Fortunately, Saradhi is not diabetic and maintains exemplary health habits, with an HbA1c of 5—well within normal limits.

Clinical Findings

On close examination, his toe was swollen, beads of pus noted and the toenail had a curved profile from side to side. The inner corner of the nail was digging into the flesh, resulting in unhealthy granulation tissue at the site of penetration. Interestingly, the granulation tissue had begun to epithelialize, giving it the appearance of new skin formation.

Treatment Procedure

Under a toe block (local anesthesia), total nail was carefully removed, and the excess granulation tissue was excised with a surgical scoop. The area was thoroughly cleaned, and dressings were applied.

Post-Procedure Care and Advice

The dressings need to be maintained for about a week, during which the nail is expected to grow back. Saradhi was advised to let the nail grow slightly beyond the tip of the toe and avoid trimming it too short, as this could encourage the nail to grow into the flesh again.

If the nail shows signs of regrowing into the flesh or does not grow correctly, surgical techniques may be considered to modify the nail bed. This may involve flattening the nail bed and slightly increasing its width to prevent recurrence.

At just 3 years old, this little boy had already shown signs of delayed speech development. While he could say basic monosyllabic words like “amma” (mother), “naanna” (father), and “taatha” (grandfather) in Telugu, more complex words were a struggle. His parents noticed that while his speech improved marginally over six months, he still found it difficult to articulate longer words.

Medical examinations revealed a tongue-tie, a condition present since birth that restricted the mobility of his tongue. This made speech development challenging as he couldn’t extend his tongue beyond his teeth. After consultations with doctors, his family decided to move forward with a tongue-tie release surgery to improve his speech capabilities.

After Surgery
The procedure was carried out under general anesthesia. During surgery, the restricted tissue holding the tongue was carefully released lateral to lateral, extending to the floor of the mouth. Over time, the treated vein is reabsorbed by the body, and blood is naturally rerouted through healthier veins.

This procedure is particularly effective for smaller veins, including spider veins, and is often chosen for its simplicity, safety, and quick recovery time.

As his condition worsened, with swelling and blisters developing, he was referred to Guntur Government General Hospital (GGH). Multiple debridements were performed there, and Satyam was eventually discharged with instructions to continue dressing his wounds at home. Despite these efforts, he ended up with extensive raw areas on his thigh, knee, leg, and foot.

When Satyam came to me for further advice, I found his wounds in a poor state. He was also anemic, despite having received four units of blood transfusions at GGH. Complicating matters further, he had a history of lymphedema in both lower limbs, which made the healing process more challenging.

After admitting him, I faced a dilemma: waiting for the wounds to improve sufficiently for skin grafting would take time, especially given the poor condition of his skin and the limited donor site availability. Both of his lower limbs had unhealthy skin with ulcerations, making it difficult to find adequate donor sites.

I decided to take a combined approach—improving the wound condition while simultaneously proceeding with skin grafting. Since I have access to a high-quality skin graft mesher, I opted to use meshed skin grafts. This technique allows for better coverage with limited donor skin.

My plan was straightforward:

1. Use meshed skin grafts to cover the knee, lower leg, and foot. Graft harvested from right thigh.
2. Leave the raw area on the left thigh, and prepare it with dressings  for a future procedure once the wounds had improved further.

The rationale behind this approach was simple. If the grafts failed, the meshed skin would still contribute to improving the wound bed. On the other hand, if the grafts survived, they would significantly reduce the raw areas and expedite the healing process.

With careful planning, I was able to expand the donor skin using the mesher and achieve coverage for the most critical areas. The procedure went smoothly, and Satyam responded well in the immediate postoperative period.

This case underscores the importance of adaptability in managing complex wounds. By combining wound care with advanced skin grafting techniques, it is possible to address significant challenges, even in patients with multiple comorbidities like lymphedema and anemia.

Recently, I had the opportunity to help a young man named Ravi, whose experience highlights how a simple, practical approach can make a big difference in addressing minor medical concerns.

Ravi grew up in a small town near Guntur, Andhra Pradesh, in a family of farmers. Determined to pursue a different path, he worked hard to secure a job in an IT company and eventually moved to Hyderabad.

Like many people, Ravi had a few lipomas—soft, benign lumps under the skin—that had been present since his childhood. These lumps are usually harmless and don’t require medical attention unless they cause discomfort or cosmetic concerns. Since they hadn’t bothered him in the past, Ravi had largely ignored them.

However, one day at work, a colleague commented on a swelling on his forearm. Though it was a casual remark, it made Ravi feel self-conscious for the first time. Smt. Surya Kumari, aged about 60 years, sustained an injury to the front part of her ankle while handling a cupboard. The cupboard accidentally fell on her ankle, resulting in a small laceration

The patient, who is diabetic, experienced worsening of the wound, which began to increase in size. As the skin started to necrotize, the doctors had to remove the dead tissue in stages during each hospital visit.

After multiple dressings, repeated removal of dead skin and tissues, and the application of powdered medications, this was the wound’s appearance when she first consulted me.

She consulted me on December 26, 2024. The wound was thoroughly cleansed, and a negative pressure wound therapy (Vacuum-Assisted Closure) was planned for her treatment.

Negative Pressure Wound Therapy (NPWT), also known as Vacuum-Assisted Closure (VAC), is an advanced wound treatment method that utilizes controlled vacuum pressure to promote faster and more effective healing. During this period, the patient maintained limited mobility as advised. Gnanamba came to me with a slow-growing mass on the back of her head. Initially, she hadn’t been too concerned, as it was painless and left untreated even as it grew larger.

One day, she noticed a small drop of fluid oozing from the swelling, which prompted her to seek medical advice. Local doctors prescribed medications at first, but they didn’t alleviate her condition. One doctor suggested it was a lipoma and recommended consulting a plastic surgeon, as the size of the mass made it unsuitable for primary closure—something beyond the scope of a general surgeon. Another doctor suspected it might be an abscess requiring drainage.

When Gnanamba came to me, I considered the possibility that the benign swelling might have become malignant, possibly due to repeated trauma from daily combing. To investigate further, I performed a biopsy. Unfortunately, the results confirmed that the swelling had indeed turned cancerous.

The diagnosis of cancer added complexity to the situation. In such cases, it is critical to remove not only the tumor but also a margin of the surrounding healthy tissue and skin to ensure the malignancy is fully excised. This required meticulous planning.

We conducted a comprehensive evaluation, including a PET scan and other diagnostic tests, to confirm there was no metastasis. Once it was established that the cancer had not spread, we developed a treatment plan. This involved surgical excision of the tumor followed by reconstructive surgery using a tissue flap to restore the affected area.

The histopathology report confirmed that the tumor was completely removed, with all margins clear of any cancerous extensions.

The procedure delivered excellent results, and once the hair regrew, the area became completely unnoticeable to others.
