<p>Mr. Saradhi, a 45-year-old businessman with an active lifestyle, presented with swelling and pain in his left great toe. Upon examination, it was clear that the corner of his toenail had started growing into the flesh, causing significant discomfort for the past two months.</p>
<h3>Patient Background</h3>
<p>Saradhi has a family history of similar issues—his father experienced the same problem. Being a businessman in a foreign country, he often travels and attends numerous meetings. His discomfort had grown to the extent that he carried his shoes in the car, wearing them only for meetings and promptly removing them afterward. In an effort to alleviate the problem, he switched to wide toe box shoes, as his feet are slightly wider than average. Fortunately, Saradhi is not diabetic and maintains exemplary health habits, with an HbA1c of 5—well within normal limits.</p>
<h3>Clinical Findings</h3>
<p>On close examination, his toe was swollen, beads of pus noted and the toenail had a curved profile from side to side. The inner corner of the nail was digging into the flesh, resulting in unhealthy granulation tissue at the site of penetration. Interestingly, the granulation tissue had begun to epithelialize, giving it the appearance of new skin formation.</p>
<h3>Treatment Procedure</h3>
<p>Under a toe block (local anesthesia), total nail was carefully removed, and the excess granulation tissue was excised with a surgical scoop. The area was thoroughly cleaned, and dressings were applied.</p>
<h3>Post-Procedure Care and Advice</h3>
<p>The dressings need to be maintained for about a week, during which the nail is expected to grow back. Saradhi was advised to let the nail grow slightly beyond the tip of the toe and avoid trimming it too short, as this could encourage the nail to grow into the flesh again.</p>
<p>If the nail shows signs of regrowing into the flesh or does not grow correctly, surgical techniques may be considered to modify the nail bed. This may involve flattening the nail bed and slightly increasing its width to prevent recurrence.</p>

At just 3 years old, this little boy had already shown signs of delayed speech development. While he could say basic monosyllabic words like &#8220;amma&#8221; (mother), &#8220;naanna&#8221; (father), and &#8220;taatha&#8221; (grandfather) in Telugu, more complex words were a struggle. His parents noticed that while his speech improved marginally over six months, he still found it difficult to articulate longer words.</p>
<p>Medical examinations revealed a tongue-tie, a condition present since birth that restricted the mobility of his tongue. This made speech development challenging as he couldn’t extend his tongue beyond his teeth. The procedure was carried out under general anesthesia. During surgery, the restricted tissue holding the tongue was carefully released lateral to lateral, extending to the floor of the mouth. The raw area was then sutured in layers from anterior to posterior using fine absorbable sutures, ensuring secure hemostasis with precision bipolar cautery. These sutures are designed to dissolve naturally, eliminating the need for removal.</p>
<p>Post-surgery, the results were remarkable. The boy’s tongue achieved excellent mobility, allowing him to extend it well beyond his teeth for the first time. This newfound range will enable him to pronounce complex sounds and words, unlocking his potential for better speech development.</p>

<p>A 9-year-old girl sustained hot water burns when her mother boiled water and placed the vessel in the bathroom. Before the mother could add cool water to dilute it, the child unknowingly poured a mug of boiling water on her chest, causing severe burns. She screamed in pain, and her parents immediately brought her to me for treatment.</p>
<p>Fortunately, they avoided applying common home remedies like Burnol, toothpaste, turmeric, or ink, which can interfere with the healing process. Although they missed the opportunity to pour cold water on the burns to minimize damage, refraining from harmful remedies was a significant benefit. I have encountered cases where simple burns that could have been treated effectively were worsened by improper treatment, leading to hypertrophic scars that can cause lifelong distress.</p>

										<content:encoded><![CDATA[<p>In November 2024, Satyam, a 67-year-old gentleman, developed a fever with chills and rigors. Initially, he sought treatment from a local RMP (Rural Medical Practitioner), who prescribed antibiotics. However, his condition didn’t improve, and he was referred to an MD doctor in Penugonda. From there, he was sent to a medical college hospital in Eluru, where he was diagnosed with cellulitis.</p>
<p>As his condition worsened, with swelling and blisters developing, he was referred to Guntur Government General Hospital (GGH). Multiple debridements were performed there, and Satyam was eventually discharged with instructions to continue dressing his wounds at home. Despite these efforts, he ended up with extensive raw areas on his thigh, knee, leg, and foot.</p>

<p>When Satyam came to me for further advice, I found his wounds in a poor state. He was also anemic, despite having received four units of blood transfusions at GGH. Complicating matters further, he had a history of lymphedema in both lower limbs, which made the healing process more challenging.</p>
<p>After admitting him, I faced a dilemma: waiting for the wounds to improve sufficiently for skin grafting would take time, especially given the poor condition of his skin and the limited donor site availability. Both of his lower limbs had unhealthy skin with ulcerations, making it difficult to find adequate donor sites.</p>
<p>I decided to take a combined approach—improving the wound condition while simultaneously proceeding with skin grafting. Since I have access to a high-quality skin graft mesher, I opted to use meshed skin grafts. This technique allows for better coverage with limited donor skin.</p>

<p>My plan was straightforward:</p>
<ol>
<li>Use meshed skin grafts to cover the knee, lower leg, and foot. Graft harvested from right thigh.</li>
<li>Leave the raw area on the left thigh, and prepare it with dressings  for a future procedure once the wounds had improved further.</li>
</ol>
<p>The rationale behind this approach was simple. If the grafts failed, the meshed skin would still contribute to improving the wound bed. On the other hand, if the grafts survived, they would significantly reduce the raw areas and expedite the healing process.</p>
<p>With careful planning, I was able to expand the donor skin using the mesher and achieve coverage for the most critical areas. The procedure went smoothly, and Satyam responded well in the immediate postoperative period.</p>

<p>Recently, I had the opportunity to help a young man named Ravi, whose experience highlights how a simple, practical approach can make a big difference in addressing minor medical concerns.</p>
<p>Ravi grew up in a small town near Guntur, Andhra Pradesh, in a family of farmers. Determined to pursue a different path, he worked hard to secure a job in an IT company and eventually moved to Hyderabad.</p>
<p>Like many people, Ravi had a few lipomas—soft, benign lumps under the skin—that had been present since his childhood. These lumps are usually harmless and don’t require medical attention unless they cause discomfort or cosmetic concerns. Since they hadn’t bothered him in the past, Ravi had largely ignored them.</p>
<p>However, one day at work, a colleague commented on a swelling on his forearm. Though it was a casual remark, it made Ravi feel self-conscious for the first time. Wanting to address the issue, he consulted a plastic surgeon in Hyderabad. After the consultation, he was informed of the procedure’s cost, which was far beyond what he was willing to spend.</p>
<p>During a visit to Guntur, Ravi shared his concerns with someone who recommended my clinic. When he visited me, I carefully examined his lipomas and explained their nature to him. I reassured him that lipomas are common and harmless, and there was no need to remove all of them unless they were causing physical or emotional discomfort.</p>

<p>We decided to remove only the most noticeable and bothersome ones. The procedure was simple and performed under local anesthesia. I used a very fine insulin syringe for the anesthesia to minimize discomfort, and the entire process involved small incisions of about 1 to 1.5 centimeters. Despite the tiny cuts, even the larger lipomas were removed effectively.</p>

<p>After the surgery, I advised Ravi to focus on removing only the lipomas that truly troubled him and not worry about the others. If needed, we could address more in the future.</p>
<p>This case is a good example of how a straightforward and cost-effective approach can address a patient’s concerns without unnecessary complexity. Ravi left the clinic relieved and satisfied, appreciating the minimal pain, small incisions, and affordable treatment.</p>
<p>For those dealing with lipomas or similar concerns, it’s important to know that these conditions are usually harmless and manageable with minor procedures. Addressing the patient’s concerns while keeping the treatment simple and accessible is what makes a real difference.</p>
